Step 1: Prerequisites:

• A head & shoulder size photo of yourself (most recent) - Selfie is sufficient

• Photo must be without any headwear.

• No sideview photos.

• Photo size not to exceed 10mb

Step 2: Completion:

• Complete all the fields for each page (there are 4 pages).

• At the bottom of each page is the "Next" button to go to the following page.

• The last page requires you to upload your photo.

Step 3: Uploading your photo

• Click on the word "Browse"

• Locate your photo and up load - wait for the "Upload complete" notice to appear.

• Click the Submit button - Wait for notification message

• Close the webpage - All done!
